Course description: To improve care quality and curb the costs, healthcare is undergoing a major shift from volume-based to value-based care. The changed healthcare landscape offers exciting opportunities for nurses to take on emerging practice roles in population health management (PHM). The PHM is a proactive approach to care, which requires education across several QSEN competencies: interpreting population panel reports, risk stratifying population panels, identifying population care gaps, and working with interprofessional healthcare team members to select and implement evidence-based interventions to close population care gaps for key quality measures. Being PHM practice ready will make nurses value-generating health care team members, which is critically important for the emerging value-based care paradigm.
