In an Age-Friendly Health System, every older adult gets the best care possible, is not harmed by health care, and is highly satisfied with the care they receive.
The intention of CATCH-ON Community Health is to bring together the key elements from evidence-based programs for managing multiple chronic conditions among older adults in and around primary care and create linkages to community aging resources.
Johns Hopkins Nursing
Teach-back is a way to confirm that you have explained to the patient what they need to know in a manner that the patient understands. Patient understanding is confirmed when they explain it back to you. It can also help the clinic staff members identify explanations and communication strategies that are most commonly understood by patients.
e-Pulmonology Review is a monthly CE series for nurses, nurse practitioners, and others which provides up-to-date information directly relevant to clinical practice for clinicians interested in pulmonology.

Building on the work of a large and growing community, the Agency for Healthcare Research and Quality (AHRQ) defines a medical home not simply as a place but as a model of the organization of primary care that delivers the core functions of primary health care.
Primary care practices are achieving this quadruple aim by creating more effective practice teams and becoming certified as Patient-Centered Medical Homes.

The National Collaborative for Education to Address the Social Determinants of Health (NCEAS)
The National Collaborative for Education to Address the Social Determinants of Health (NCEAS) seeks to accelerate the spread of the best approaches for identifying, understanding, and addressing social determinants of health by preparing the healthcare workforce. Their mission is to understand the complicated web of factors that lead to unjustly disparate health outcomes in poorer and more disadvantaged communities. The NCEAS team is united behind the belief that preparing healthcare practitioners to understand social determinants, and learn skills and approaches to addressing them, will improve the health of vulnerable individuals and communities. The NCEAS site contains research, toolkits, webinars, games, curriculum and curricular evaluations that can be used by health educators, healthcare professionals and students, and the general public.
